Summer is here and so is "camp & school forms" season. If your child needs forms completed and has NOT had a physical within the past 12 months, please call ahead and make an appointment. Bring the forms with you to your physical. If your child has had a physical within the past 12 months, he does not need another one. Just mail in the forms and we will complete them. Also, please review our policy under "Our Services--- Forms Request Policy" before you submit your forms. Thank you in advance.

We are out of seasonal flu shots, but we still have a large number of H1N1 flu shots. If your child needs the H1N1 shot or booster, please come in on Tues, Wed, or Thurs afternoons from 1-4pm. These "shot-only clinics" are walk-in and no appt is needed. We will continue to give out these vaccines until the supply is gone. Thank you very much.
