Office hours:
The office is open M-F from 9am – 5pm and Saturday from 9am – noon. There are typically
4 to 5 doctors in the office during weekday hours, but every attempt will be made
for you to see the doctor of your choice. Saturday office hours are shared by the
physicians in the group on a rotating basis. As there is only one doctor in the
office on Saturday, we can not perform routine check-ups or deal with behavior/discipline
issues. Our policy is that we see patients during office hours by appointment only.
This lets us adhere to the schedule and allows each patient to see their preferred
physician. Walk-ins will be handled on a case-by-case basis but we discourage this
practice. If you are going to be late for an appointment or need to cancel, please
call our main office number.
To reach us during office hours please call 404-351-6662. Calls are answered by
the staff, who will attempt to answer your question and/or give you an appointment.
Questions which cannot be answered by the staff will be forwarded to the patient’s
physician. Urgent calls will be handled immediately. Non urgent calls will be returned
by the physician as soon as possible.
After Hours and Weekends:
If you have questions after hours, you may call 404-351-6662. An operator will answer
your call and obtain the needed information. It is assumed that the type of problems
leading to after hours calls are not of a life threatening nature. If your child
is experiencing a life threatening illness, please call 911. For routine after hour
calls, you should receive a call back within one hour from a Children’s Healthcare
of Atlanta nurse. She will screen the call and answer your question or forward it
to the doctor who is on-call from our group.
For patients whose problem is serious enough to be seen outside of routine office
hours, we encourage you to use Children’s Healthcare of Atlanta at Scottish Rite
or Egleston.
Prescription Refills:
Prescriptions can be refilled on the same day if received by our office by 4pm.
You may print a copy of our refill request form and fax it to our office. It is
very important that you include the name of the medication, strength, dose the child
takes, and the frequency with which it is taken, as well as the pharmacy number.
For asthma medicine, please specify the delivery method (such as nebulizer, metered
dose inhaler, etc.) Failure to provide this information may cause a delay in filling
the prescription.
ADD medications can not be phoned in to a pharmacy. You must present an original
prescription to the pharmacist. We ask that you give us at least 24 hours notice
for a refill on ADD medication in order to allow the staff to obtain the patient’s
chart and the doctor to write the prescription. Prescriptions will be left at the
registration window for pick up or can be mailed if you have provided us with self
addressed stamped envelopes.
We do not fill new prescriptions or make diagnoses over the phone. It is important
that the child comes to the office so that an accurate diagnosis can be made to
determine if a prescription is necessary to treat the condition.
New Patient Forms:
Prior to your first visit, please print and fill out a copy of our new patient form
and bring it with you to the office. It is also very helpful to have a current copy
of the child’s immunizations and growth chart. Copies of your child’s records can
only be requested by you. We have a request for transfer of records form which you
may print and sign and mail to your previous pediatrician.

Forms Request Policy
Due to the extremely large volume of requests for medical records, hearing, vision,
immunization records, school, sport, and camp forms, it has become necessary to
implement a written policy for such requests. You may request any of the above listed
forms to be completed for your child by calling our office at 404-351-6662, faxing us
a written request at 404-351-6030, or requesting at your office visit.
We have established an Annual Administrative Fee of $10.00
per child or $30.00 per family for completion of forms that will need to be paid
prior to completion of forms request. This fee is not covered by your insurance
company and in part covers the retrieval, administrative costs, and physicians’
review of your records and/or forms.
If you require a copy of your medical records, the first copy will be provided at
no charge. Any additional copies will be charged according to the Georgia code.
Forms are completed on a ‘first in/first out’ basis. Please allow 5 business days
for your request to be completed. We make every effort to complete these forms as
quickly as possible. Due to the heavy volume of requests during back-to-school months,
processing times may be longer. Please plan ahead!!
Parents MUST complete all information on the forms that they have knowledge of (i.e.
demographics, health history, etc.), and MUST sign and date their portion of the
form. Incomplete forms may delay the processing of your forms.
Parents will be required to provide self-addressed, stamped envelopes or pick up
the forms at the office. Due to HIPPA Privacy regulations, we will no longer return
completed forms via fax.
